New Patient Registration
We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one.
Anyone wishing to register as a patient with the practice and who resides within the practice area should speak to the receptionist for completion of the appropriate forms. The documents are also available to print off below.
All new patients are required to attend for a health check with the healthcare assistant at the time of registration. It would be helpful to bring ID documentation with them including proof of address and photo ID if available.
Families are encouraged to register at the same practice wherever possible.

Please ensure the forms are fully completed to avoid any delays to your registration.
The practice accepts patients from Maltby, Hellaby, Carr, Hooton Levitt, Stone and Firbeck areas. The catchment area for this practice is shown below. To check if you are eligible to register at this practice please enter your postcode in the box underneath the map.
Medical treatment is available from the date of registration. Please contact reception for further information.
Change of Address and Personal Details
You should inform the practice if you change your address or any other personal details (i.e. name, telephone or mobile number) in order that we may keep your medical records up to date. If you move out of the practice area you will be requested to seek registration with a local GP in the area where you live.
